Beginning in February 12, 2007 Boeing IDS Wichita will provide Rx prescription safety glasses to it's employee's per PRO-5631 "Integrated Defense Systems Safety, Health and Environmental Affairs (SHEA)" - Exhibit A - Prescription Safety Glasses.
IDS will pay the cost as follows:
All prescription safety glasses lenses will be polycarbonate and side shields will be permanently affixed.
Eye examinations are the responsibility of the employee (insurance) and will not be paid for by IDS. Employees are required to provide a current prescription (less than one year old).
Prescription safety glass frames may be
replaced every 2 years unless damaged or defective and approved by the site SHEA
office.
Replacement lenses are provided whenever an employee's prescription changes.
Frame Options: There are seven different groups with several different types of frames in each group that the employee's may choose from depending on which style they like best. Choose from the Basic, Economy, Fashion, Deluxe Collections and Premium Groups 1, 2 and 3.
Check to see if your eye care provider is one of the 58 locations in Wichita and the surrounding area who are participating in this program. If not just use your current prescription that you get from your provider when you have your regular eye exam (that your insurance pays for) and take it to any of the 58 participating providers to order the safety glasses that you like best from one of the seven approved groups (listed above).
To start this process: make sure you read and are familiar with PRO-5631 Exhibit A - Prescription Safety Glasses, then go to our on site Medical facility in building 4-118F to pick up an AuthorizationForm. You will need your BEMS ID # and your correct, up to date organization number in order to receive an Authorization Form.
This is to ensure proper billing to the correct budget. Employee's will need this form to order your prescription safety glasses from whichever provider you use. (Sample on next page)
IDS will not pay for prescription safety glasses without a properly completed Authorization Form.
